I agree to abide by the policies and procedures surrounding the use of 3D Printing equipment and services, hereafter collectively referred to as “3D Printing Services”, provided by the Fort Frances Public Library Technology Centre (FFPLTC). I agree to the terms of this agreement on behalf of any children I can legally represent, if indicated below.
My use of 3D Printing Services entails known and unanticipated risks that could result in physical injury, or damage to myself, to property, or to third parties. I understand that such risks cannot be eliminated without jeopardizing the essential qualities of the activity. The risks include, among other things: exposure to burns, eye damage from flying material, or particulate inhalation.
FFPLTC staff seek to create a safe environment but may be unaware of a participant's health or abilities. FFPLTC staff may give incomplete warnings or instructions. 3D Printing Services may malfunction. I expressly agree to accept and assume all of the risks existing in this activity. I am responsible for restricting access to 3D Printing Services provided to me by the FFPLTC to my own personal use. My participation in this activity is purely voluntary and I elect to participate in spite of the risks.
I release and hold harmless the FFPLTC, their board, staff, volunteers, and committees from and against any and all liability, loss, damages, claims, or actions (including costs and lawyer’s fees) for damage to myself or my property, arising from my use of 3D Printing Services.
I agree to indemnify the above parties for any liability, loss, damages, claims or actions (including costs and lawyer’s fees) brought by a third party arising from my use of 3D Printing Services.
If any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.
